Urgent required document to send for OPT extension

I applied for OPT extension and i got RFE for sending more document. The following document which i have to send is:

1)Notarized statement from your employer attesting to your employment with the firm

2) i worked for unpaid intern for 3 month but they require

a) copy of employment agreement with start and end date

b) copies of completion certificate (which i do not have)

c) copies of company time and attendance records that document(which i do not have)

d)Notarized statement from your employer attesting to your employment with the firm

My question is i don't have b and c document so what should i do for that and how do i process point D . I have to tell my employer to notarized it or should i take a letter and notarized by my own..

Why do you not have copies of your attendance time sheets? Your company should be able to provide copies if you filled them out or used an automated logging system.

I do not know what is meant by "completion certificate".

Your company likely has a notary on their staff. They - not you - would have it notarized. That is to prevent fraudulent submission of documents by the applicant.

Can u plz give me an update about ur status. What all docs u have submitted?
