While entering details in DS160 application form online, there is a

Travel Information section. In this section, what is to be entered in the Principal Applicant Information

Surnames: ??? (Whose name goes here ? The name of the dependent applying for H4 visa OR should I enter the name of the person who is on H1B and supporting the H4 visa applicant ? )

Given Names:

(the dependent applying for visa OR the person who is on H1B supporing the dependent ? )

Application Receipt/ Petition Numbers:

Pls provide your inputs ASAP.

There are two places where you enter the names.

One at the top where it says - Name: That is H4 applicant's name.

And then after you select the visa category as H4, you will see Principal Applicant's name boxes - you need to enter H1 holder's name in the surname and given names section. I guess that is what you are referring to.
