In this section, what is to be entered in the Principal Applicant Information Surnames: ?????? (the name of the dependent applying for visa OR should I enter the name of the person who is on H1B and supporting the H4B visa applicant ? ) Given Names: (the dependent applying for visa OR the person who is on H1B supporing the dependent ? ) Application Receipt/ Petition Numbers: Pls provide your inputs ASAP.
