EAD/AP renewal, last entry using AP

Gurus,

I have Valid H1B till september, 2014. My EAD/ AP are expiring soon. I have not used my EAD. Last month I travelled to India and used AP for entry.

My new I-94 has "Paroled" stamp and "AOS" written on it.

Please help me out to fill following questions,

question 3 "Class of admission"

I 765

Question 14 manner of last entry

Question 15 Current immigration status

My Answer: "H1B" and for my wife I mentioned "H4"

I received EAD/AP within 15 days from NSC.
