Regarding I-134 form for visitor visa

Hi,

I am currently on my H-1B visa and I am planning to invite my parents (both mom&dad) and my grandmother to visit me. While filling in the I-134 form, I found the question no.3, where there is a box to provide the details for whom the sponsor is (ex, i mention my father's details) and there is a box for the spouse of the person mentioned (ex, i mention my mother's details) and for children. But I am not sure where I should mention my grandmother's details. I am confused whether I should fill in 1 form (for father, mother and grandmother) or 2 forms (1 for father&mother, other for grandmother)? They are all planning to sit together for the visa interview.

Please let me know your thoughts.
