DS 160 Form Questions

Hi,

Please excuse my ignorance but I have a few questions on filling the DS160 questions -

1.Has anyone ever filed an immigrant petition on your behalf with the United States Citizenship and Immigration Services?

Does this refer to I140 or I485 application?

2. The DS 160 form is asking if I have been issued licenses at any time. I probably have 3-4 (old) licenses issued by different states. Do I need to give the information for all of them?

1) i had my I140 approved . this is how i answered it. "I got my I140 approved from XXX company" .

2) your current license info is enough.
