Birth Certificate for my wife

My wife has a birth certificate. but her certificate has a couple of issues. Please help how can we resolve these issues.

1. Her name is not written on the certificate.

At the time of registration,Naming Ceremony was not held so name is not present on the certificate.

2. Her parents name spelling is wrong.

Actually No One from her family is currently staying at her birthplace so it is very hard for her parents to make a new birth certificate.

Can above two problems be resolved by affidavits?
