Re-entry permit and AR-11

I applied for re-entry permit and my application has been approved. I had directed my reentry permit to be sent to the London consulate and I will pick it up from there when I arrive there in a weeks time.

My question is this: Do I also need to file AR-11? The form does not have a provision for a non-US address.

I can provide my relatives address in US but do not prefer that. Kindly advise.
