prev emp dates on ds160

Hello All,

please help me fill ds160 form for h4visa.

earlier i am on h4 here and my h1b approved from oct. due to medical reasons my payroll started later in feb. For previous employ start dates, should they be when the employer start paying or will it be h1 approval startdate?

What dates should I mention on DS160? start date should be Oct or Feb?
