Urgent: previous H1b employment dates for DS160 form

Hello All,

please help me fill ds160 form for h4visa.

earlier i am on h4 here and my h1b approved from oct 2011. due to medical reasons my payroll started on feb 2012.

for previous employment start dates, should they be when the employer start paying or will it be h1 approval notice startdate?

What dates should I mention on DS160? start date should be Oct 3rd or Feb 1st?
