L1 to H1 clarifications

I was in India. H1 approved in Oct 2006 (company A), never entered to US using that H1, visa was rejected in that H1(H1 expired oct 2009).

Got L1 in Sep 2011(company B).Entered USA worked on L1.

Filed COS L1 to H1 using old H1approval notice, cap exempt(applied Aug, 2012, In that old H1, 6 yrs is there until Oct 31st 2012).

My question.

1)Do I need to wait to work for company B untill the H1 is approved? or once I get the receipt notice I can start working for company B?

2) If the company A would have revoked my H1, Will I get cap exempt for L1-H1 COS?

3) if COS approved(still it is pending), Do I have remaining 6 yrs in H1?
