Applying for Canada Visting Visa for H1B stamping

Hello,

Following are the questions, I have on my Canada visiting visa process. Appreciate if any of you can answer this.

1. I did completed, the IMM 5257 (TRV), but under Employment, do I need to add the client name or the my employer name or it does not matter.
   
2. Completed IMM 5645E (FAMILY INFORMATION).
   
3. Are there any other form do I need to submit ?
   
4. I think I should send my original passport to below address. But what does this means “We do not recommend you bind or place your documentation in plastic pockets; usually an organized pile with a large elastic band around it is suitable”. How should I send it to the Consulate.
