DS 160 Questions-Please advise

H1-H4-H4 renewal stamping

I was on H1b and changed my status while in US to H4.I applied for the renewal of h4 and planning to go for stamping in india next month

While i was on h1, my employer applied i140 petition but it was denied based on the financials of the company

I am trying to fill DS 160 form and not sure what to mark for the following questions.can some one please advise?

Have you ever been issued a US Visa?

Should i give the details of H1b here or h4 as it was my last visa status and now i am going for the renewal of the same (but i only have h1b stamp in my passport not h4 as i was not out of the country since COS)

Has an immigration petition filled on your behalf?

As i140 was filied for me,I think the answer should be YES for this question but not sure...should it be NO as it was denied ?
