H1B Marrying a US citizen in India

Here is my situation -

* Current Status - H1B stamp (expired), I797 valid till 04/13/2013.

* I140 Cleared.

I am marrying my fiance, who is a US citizen in Nov first week in India. My visa is not stamped on my passport so i am wondering if it might be a problem getting it stamped in india.

Questions -

1. Should we get married here in a US court and then go to india and do a full wedding?

2. Shall i start the paperwork straight away or is it beneficial to wait after getting married in india?

3. How much time does it take after getting married to get an EAD?
