H1-H4-H1 Query

I got my new H1B in 2009 when I was in india and valid till Nov 2011. I came to US on H4 visa Nov 2010. Till now I was in H4 visa.

Now I got a employer who is ready to sponsoring my H1b transfer

is it eligible to get H1b(not on quota) from H1-H4-H1 without even worked for company A(who inititally sponsored H1)?

Since I didn't work for my first H1B, do I need to submit paystubs while I transfer from H4 to H1 ?

experts - please advise.
