I140 approved. Need LCA for extension?

My Labor Certification and I140 for GC have been approved. I485 not applied. I have two questions:

1. Is a new LCA (Labor Condition Application) required for H1b extension?

2. My H1b 6th year expires February 14th 2013. Should I wait to get closer to the expiration to apply for the extension or apply now? I have read many posts about applicants only getting 1 year extensions even though they were eligible for 3 years.

2. you can apply extension after 6 months or less are left for expiration, so you can apply after Aug 14 2012. Generally extension is 3 years for approved 140.
