Do I come under H1 cap ?

Hi,

I did my MS in USA and later on worked on H1 for 2 years and came to India for my marriage in Oct 2010. After coming to US I never visited India so as my status changed to H1 I went for stamping and my visa was rejected. my visa was from oct 2008 - Sep 2011.I am here in India since oct 2010.Now I am planning to come back to US on f1 or H1 ..I have two questions.

1) If I apply for H1 do I come under H1 Cap as I already have H1 once..?

2) If I get an I20 from an University for this fall 2012 and If I got stamped..Can I enter US for spring 2013 or fall 2013 without going for visa stamping again?
