H1B to H4, and back to H1B

I was working in H1B till April 2012. I applied for COS to H4 and got it approved. Now, I have a company offering my H1B. Please elaborate the documents and the procedure needed to be filed to USCIS.

Also, my husband's H1B is under extension process (along with my H4). If I apply for my H1B now with the company that is offering me a job, I will get my H1B before his extension is approved. Will my status go back to H4 if his H1-B extension is approved (as my H4 is tied to his H1B)?

It would just be a new H1 not under the cap. The same documents as for every other H1 are required.
