i-94 stamping

Hi,

I have an H1B visa which has validity till Oct 31 2011 and it got extended and new card is valid from Oct 31 2011 to 2013.i changed the job to another company as a Full time employe and got visa which is valid till 2015.

i am going to india on july 21st, what are the documents i need to carry for the visa stamping.

is there any document checklist for visa stamping ?

doing visa stamping in canada is good?
