H1B visa stamping in India but unable to select any date

I am in USA and I am planning to go to India for H1B visa stamping. I have few questions.

1. I saw I need to carry copies of the I-797, I-129 and other petition-related documents H. Should I fill out I-129 form or should I get it from my employer?

2. I need to mention “Address and phone information” in DS-160 form. As I live in USA, should I mention my USA address or my permanent address in India?

3. I was trying to book one visa interview date in Calcutta, India. I visited https://www.vfs-usa....fResidense.aspx. I saw few dates are available (i.e. dates with green color) and I saw following message on the page:

The page is active for 5 minutes only. If you do not continue, you will be timed-out. You will need to re-enter your information again.

But I didn’t see any continue button I saw just “exit” button. I visited same web site for few times but I have never seen and “continue” button. How to book a visa date in India?

It would be really helpful if someone can give me some ideas.

1. You have to get the I129 from, used when filing ur H1, from ur employer
