Green card through marriage

About me:

I have 2 more years in H1 B and my I 140 is approved, waiting for i 485 priority date.

My fiancee is a US citizen and we are planning to get married in the month of Nov of this year in India.

Question:

1. My fiancee and i are planning to get a court marraige in the month of May and apply for green card. How long will it take for this process to complete? can we still go to india in november to have a cermony? what happens if we get the inteview before the wedding in november?
   
2. Does it matter if we are only getting court marriage before the ceremony?
   
3. My fiancee will be continuing her medical education at a different state than where i reside for the next 2 years, does this matter when applying for the green card via marriage?
