H1 extension beyond 6th year

Hi,

I am a physician currently in 5th year of H1 and am doing a fellowship that ends in July 2013. My 5th year ends in June 2012. I havnt been able to find a job as of now and getting a good job by june of this year does not seem realistic.

So If I am not able to find a job and file my I140 by end of my 5th year , what should I do to stay in US?

Is there anything I can do in 6th year that can give me extension for another year. I am sure I ll get a job and green card filing by next year, but will it be too late??

Will appreciate any input on this.
