Regarding my H1B eligibility for extension

1.I have completed 6yrs of my H1B in June 2010 and Got I-140 Approval from Company A in April 2010.Based on I-140, i have received H1B extension Approval till 12/31/2013 from Company A.

2.I have Transfered my H1B to Company B in June 2012 and got only 1 year extension till 06/05/2012.

3.Company B filed PERM in Oct 2011.

4,Got Audit of PERM in FEB 2012.

Can I go for H1B extension with Company B right now with out PERM approval and 1-140.

I would like to know what are challenges I have with H1B extension? ASAP.

Can I get H1B extension on I-140 Approval from Comapny A or already have H1B approved from Company A (which might have been revoked by Keane).
