L1B/H1 6 years limit

1.I am on L1b visa since jan 2007, and it expires in nov 2012, and my employer is not doing GC. My wife is applying for medical residency and she has an option of H1b vs J1. What are our options if both of us want to continue working.

2.If my employer does H1b for me, that gives me 1 more year to work. After this 6 year period, if I want to re apply for H1b, I know I have to wait for an year. Is it necessary that I go outside of US during this one year or can I be on H4/J2(as a dependent) and apply H1b after this year?

3.Are there any limitations to work while on J2 visa? How would this be different from work on H1b/L1b?

4.If I am on J2, can I change to other visa types?

Is it for the same visa or either - i mean - if your 6 years is over on H1 - if you want new H1 - one year outside the country for sure -

but what if 6 years is over on H1 - applying for a new L1B ?

Yes.

When you have reached the maximum time, you have to be outside the US for one year before you can get another H1 or L1.
