H1 extension denied, 1-140, what to do next?

I have a approved 1-140 since March 2009. I filled for H1 extension, got a RFE and now it is denied. What are the next steps for me?

1) Can I apply for fresh H1 based on my i-140 approval. If I do premium processing, how much time will it take?

2) Should I do a MTR. what are the chances of approval in this case?

With premium evrything should be done in 3-4 weeks

2. Mtr takes upto a year.
