L1B - L1A Extension filed - Got I797 B with approval

I was on L1B and my petition and I94 expired 3 months back. My employer filed extension and L1A amendment. I got I797 B approval notice without I94 after 10 days of my I94 expiration. There was no reason mentioned in I797 or RFE on why my I94 was not received. When I talked to my Immigration team, they told it must be in error and sent a letter to USCIS to correct the error. Its been 3 months I did not get any answer from USCIS. When my immigration team contacted USCIS, they said it may take atleast 60 more days since they have to retrieve the files to do the correction.

Now, my questions are:

1) Are these cases normal and generally how long does it take to resolve these errors?

2) My spouse is on H1, Can I change my status to H4 now while I am in US. (Note: My I94 is expired)

3) Is there any way to expedite this process like premium processing?

Is L1B to L1A considered as COS?

I am Indian national, Can I go to Mexico to get the stamping done? (I dont have I94)

I read in the website http://nogales.usconsulate.gov/tcn_visas.html that if we change the visa category we cannot go to Mexico. IS L1B to L1A considered as different visa category?

Please help! Your answers will help me make some important decisions!

Also do we need VISA to travel to Mexico for stamping. If yes, since I dont have I94 will I get the Visa?
