Hello Friends,

I got my I-140 Approved.

Want to know what all documentation is required from employee for filing I-485 Adjust Of Status.

Appreicate if you can get back to me asap.

One of the most important document is your birth certificate. If the certificate was not issued within 1 year of your birth, you better get a affidavit from India. Your attorney should be able to provide you more details about the documentation needed.

· Form G-1145, E-Notification of Application/Petition Acceptance

· Payment in the amount of $1070.Check payable to US Department of Homeland security

· Photos, 6 count

· Form G-1145, E-Notification of Application/Petition Acceptance

· I-485 Application to Register Permanent Residence or Adjust Status

· I-765 Application for Employment Document

· I-131 Application for Travel Document

· Form I-693, Medical Examination of Aliens Seeking Adjustment of Status (In Sealed Envelope)

· Form G-325A, Biographic Information Sheet

· Copy of approved I-140 under EB2 category

· Copy of Approved Labor Certification from Department of Labor

· Copy of Passport

· Copy of Visa in the passport for last entry into the US

· Copy of I-94 for last entry (back and front)

· Current Status: H1B approval notice

· Original employment letter from the employer

· W2 from 2008, 2009, 2010

· Last 4 paystubs Oct 2011, Nov 2011, Dec 2011, Jan 2012

· Copy of birth certificate

· Original Affidavit supporting birth certificate

Support Documents

· Previous H1B approval notice

· Old passport copy

· Old non immigrant F1 student visa

· Optional Practical Training Approval (OPT)

· Copy of OPT card
