I94 Received with Gap and 485 filing

This is the background.

H1 Extension was applied on Sept 10^th 2009. H1 and I94 were valid till Sept 30^th 2009.

Moved to another company on Nov 15^th 2009 while H1 extension still under process. New company filed for H1 Transfer on Nov 17^th 2009. Receipt Date was Nov 19^th 2009. Started working for them from 20^th Nov.

Did not like the work at new company due to unexpected traveling every week and decided to come back to my old employer. My old employer told me that H1 extension filed in Sept 10^th 2009 was approve on Feb 2^nd 2010 .Joined back old employer on 02/15/2010.But, later when i saw the approval notice I94 was issued valid from 02/03/2010 to 09/30/2012 and not from the oct1st 2009.

Below are my questions.

1. There is a Gap in the I94 for 122 Days.
   
2. Worked for a company after filing the H1 Transfer while other extension is pending and moved out of that company before approval or denial decision is made.
   

Would this cause any issue during 485 stage ? if so, any suggestions on how to proceed.

Will get in touch with attorney but would like to have an second opinion.
