Query regarding difference in wife's name in passport and I797 after name split

I had a query regarding my wife’s (H4) name as per the passport and as on I-797.

My wife’s name as on Passport :

First Name: Lakshmi Venkat Eshappa Last Name: <<blank/unknown>>.

This time when I got the new I-797 after renewal of Visa some 6-8 months back , her name reads as “Name of Applicant : LNU, Laksmi Venkat”.

Her Previous visa stamp had : Surname: Lakshmi Venkat Eshappa First name : FNU.

Since we had lot of problems with the last name not being there. I recently made the Indian Consulate note in her passport saying that “Venkat Eshappa “ is the last name and first name is Lakshmi . I am going for stamping sometime in the last week of December or Early Jan. If I fill in First name as “Laksmi” and Last name as “Venkat Eshappa” , Will there be a problem since I-797 has LNU as the last name. Has anybody faced such a problem before, any reply will be extremely useful.

Also in case we obtain the stamping as First name: Laksmi and Last Name : Venkat Eshappa …will that be a problem at the I-485 stage since the Visa and I-797 document do not match. Requesting you to please reply at the earliest.
