H4 Visa - 221(g) YELLOW SLIP

Hi

My Wife applied H4 Visa in this week and she got an 221(g) YELLOW SLIP at Hyderabad Consulate on that form VO marked 4 options

1) A Copy of the Petition with all supporting documents as filed to USCIS

2) State Unemployment Wage Reports showing all wages paid to each employee.....

3) Your US Income Tax returns and w2 forms

My Questions:

1) For 1st query what are all supporting docs i need to submit

i already sent to I129, LCA to my wife at her visa interview time..what more supporting docs i need to submit for this query

2) For 2nd Query ...my employer has only 3 employees out of 3.. 2 employees are corp-corp and one H1b (That is Me) and my H1b started in Oct 2011 and my wife applied H4 visa on Nov 2011 and my employer saying you started your employment from OCT 2011..so you wont get Unemployment Wage report ...

So in this case what docs i need to submit for this query

3) For 3rd Query...I started my employment from May 2011 so i don't have my W2 forms...in this case what docs i need to submit for this query

I would really appreciate if i can get clear picture on this..as i need to respond ASAP
