H1B stamping future date

Hi,

I am travelling to India in December and have to go for stamping as my visa is expired already. Now, the problem is my current I-797 is valid only till 04/02/2012 and I got a 3 year extension (as my I-140 got approved) from 04/03/2012 to 04/02/2015. I have two separate approval forms for both of them. While booking for appointment, it is only asking for one receipt number, I am not sure which one should I mention as both are valid now.

Could some one please advise me on this ASAP? Need to book my appointment.
