Please Guide me to file Adjustment of status and AP

Dear Sir/Madam

Please guide me your valuable advice to file my adjustment of status and AP.

My wife just become US citizen. I have approved I 130. now It time to apply my rest process with form I 485. Please can you advice me what forms should I have to submit with I 485?

As my Knowledge, I have to send, I 485, I 765, I 864, I 131.... should I add any more form? should I have to fill medical exam form ( i read somewhere) My OPT expire on 03/01/2012.

Please guide me your advice so I can fill out all application
