H4 Approval Notice

Hi,

I have a quick question. Could you please answer me ASAP.

I recently applied for H1 and H4 extensions thru PP and got approved. Due to the recent USCIS mailing change My employer received my H1 approval document. But i haven't got H4 approval document yet. I called USCIS to find out what address do they mail. But rep couldn't find the address.

I moved from Philadelphia to NJ last year. And i didn't notify USCIS thru AR-11. But when i apply for extension this year, i specified the current address only.

So my question is

1. will USCIS send the H4 Approval notice to the applicant based on the address on the application or what ever address they have on their file?

2. In case if they sent to my old address what should i do? USCIS rep suggests to do Address change and request a duplicate copy. If i request a duplicate copy will it have I-94 attached to it or not?
