Lost H4 extension notice

I filed for my Wife's H4 extension in Aug' 2010. through my company's official attorney form but paid myself as my company did not cover for dependents immigration costs.

When i check the USCIS website it always shows me the case was in initial review status. When i check with attorney in early January 2011, they said it may take another 3-4 weeks. And i checked again in march, this time they called USCIS and told that there was a wrong address in file and they updated correct one and may take another 2-3 months.

Finally they came back and said that USCIS might have sent the document in last Aug 2010 to my home address and it might be lost in mail and I need to file I-824 to get a duplicate copy and I need to pay for the fee.

First of all, i never received any documents to my home address, it was always mailed to attorney address,

- Do i responsible for this mistake?

- why should i pay for getting duplicate copy?

- how to get this resolved as my wife's I-94 expired in Sep 2010 itself. All I have is the receipt for the extension filed.

please advise.
