Changed Consultants.Need to apply for wife H4

I changed my consultant in Feb 2011.But my consultant was asking for 900 dollars to transfer my wife's H4.So I thought I will do on my own.Her visas are till June 26 2011.So I need to start her H4 application process.

Appreciate if some one can let me know the procedure for same?
