working remotely after I-140 approval due to medical emergency

Hi,

I have approved I-140 (EB2 - PD is Sep 2008) and my 6 year H1-B wil complete on Sep 30th. Company has already filed 3-yr extension for H1-B

Due to medical emergency, I have to go to India and planning to work from India for 6 - 9 months. My manager has agreed on doing this.

1) Will there be any issue when I come back to US after 9 months at port of Entry?

2) Should I be on US payroll or do I have to go to India payroll?
