H4 extension I 539 filling questions

I am going to file H4 extension for my wife by myself. My H1B is already extended till 2014 and I have USCIS receipt. Below is my understanding of questions, please respond with your suggestions.

My husband got his H1b approved and i am applying for a visa extension now. 

 

If you are done with the H4 visa extension for your wife, could please let me know if these answers for the question are gud to go.

I have an H-1B.  My two daughters are 5 and 7 years old.  Can I file to extend their H-4 status using only one I-539 and list one as the applicant and then the other as a family member on the supplement?
