My spouse had a valid h1B , set to expire in May 2026. she quit her job in April 2024 and applied for a cos to h4. She is visiting India for a personal emergency and would like to book an appointment. The form has 2 questions that she’s unsure how to fill. Can someone help please? 
 
Question 1: Did your previous visa expire within the last 48 months? (Does she answer yes? )
Question 2: Was her previous visa canceled, stolen or revoked? (Does she answer yes?)
