Name change during Naturalization

Hi,
I did not ask for name change in my naturalization form(N400). But I asked for changing my last name during the interview. 

My oath ceremony was held in a USCIS Atlanta office, and I received my naturalization certificate, But I did not receive any court document/letter for my last name change.

For surrendering my Indian passport I am being asked a document that states that I have changed last name during naturalization process.

What are my options now? How can I get this name change document?

Does the naturalization certificate show your new last name? Was this a court ceremony with a judge? Name changes require a court session, with a judge presiding. If this was a court session and your new name is on the certificate you should have gotten a document about the name change.
