Is this unauthorized employment?

We applied for H4 + EAD for my spouse who is on OPT and specified future start date for Change of status(03/2024). We received approval for H4(08/23) and it has validity of my H1, which is in previous year (10/21- 10/25). We were under assumption that the COS effective date would be the date we specified in the application and continued working(EAD is still pending) . We were not aware that COS is active as of notice date. My wife stopped working immediately after we realized this could be unauthorized employment. How do we report/rectify this to prevent issues in future. Anyone who been through this, please share your experience
