WHAT TO PUT FOR "HAVE YOU EVER USED OTHER NAMES (I.E., MAIDEN, RELIGIOUS, PROFESSIONAL, ALIAS, ETC.)" IN THE VISA APPLICATION?

Hi Sir,

For an Inter country adoption done in India and child relevant documents are obtained (Birth certificate, Passport) etc. In the documents the new updated name is being listed. If the Surname/Name of the child got changed due to adoption process. Does this need to be mentioned in this case?  Please confirm.

Generally, all information should be presented truthfully in an immigration application - this would include any and all names a person legally had prior and after an adoption.
