I-485 J was submitted on my case and is approved by USCIS but I didn't file it. Is there a way to find out who submitted I-485J?

My Green card request is filed in EB3 category. Recently we received I-485J approval. Fact is, I or my attorney never submitted I-485J. Is there a way to find out who submitted I-485J on my behalf? Can USCIS do that? Fees is also waived off. We are not certain who initiated this request. Will this cause any issue in GC

Generally if an application was filed / approved in your name, then you can request a copy of such documents by making a Freedom of Information Act Request to the agency.
