H1 - H4 Urgent!!!!!!!!!!!!!! Please advice

Hello All,

Please advice me on the following.

I am working on H1 since 2008 and is valid till Sep 30'2011.Initally i was planning to travel india to get my status changed to H4.Due to my health conditions doctors suggested not to travel.My husband is on H1 and visa valid till 2012.

I am planning to apply COS[H1 --> H4]from here.

1) Just wanted to know the latest processing time.

2) Is there any premium processing for this status change(h1->h4).

3) In case if we file for I 485- what is the status i will be mentioning.
