'Cancelled without Prejudice' Stamp on my H1 Visa

I picked up my passport yesterday at Hyderabad VAC and surprised to see that my visa has been stamped but marked as "Cancelled without Prejudice".
Below are the Timelines :

Submitted Passport at Hyderabad VAC - June 22

Changed from AP to Issued on July 6

Again changed back to Refused on Jan 5,2022

Got mail to pick passport - Jan 6

I didnt receive any slip along with passport which left me in confusion on next steps.

This is my first H1 applying from india
