H1b visa renewal - 221g with only DS-5535 checked

Anyone got 221g at Hyderabad consulate for h1 renewal with only first box checked ?

First box says that you will get an email within 5 business days and to respond to that email. I got an email asking to fill in a questionnaire- Supplemental information for visa applicants which is form DS-5535 asking travel history, employment history etc.,

No documents asked and passport returned, interview date- Dec 10, 2021

Please reply if anyone in the same situation.
