H4 Extension-questions on Immigrant petition

Hi , 

Recently I have applied for Adjustment of Status (for me and my wife) (i-485 + EAD + AP) and i-140  application downgrade from EB3 to EB2. I have received receipts for AOS (for both me and my wife). No biometric appointments yet. My company is applying for my H1b extension and  H4 extension application for my wife along with it . H4 visa has about 5 months left on it. 

I am not sure what to answer on these following questions on i-539 form  :
