Date of Birth Certificate mismatch

Hello there,

My priority date has just become current. I do not have my Birth Certificate. Also, the document for Birth Certificate(i.e. School Leaving Certificate) has incorrect place name while my passport has correct name. Both these document have incorrect date of birth.

The hospital certificate has correct date and birth place but it does not match my date of birth in passport.

Please advise how should I resolve this problem. Thanks in advance.

you can submit a affidavit. get the affidavit from 2 people and submit. You should be notarized the the affidavit.
