H4 was approved 4 months Before, but H1B still Pending even after premium processing

My employer submitted H1B visa extension along with I140 approved copy (08/20). It was submitted near to I94 period (8/31). The close dates were due to my personal emergency. We submitted H4 for my spouse. The H1B RFE response was submitted by 01/14. As approval was not received, my employer submitted our premium processing by 03/14. Between we called uscis by 3/11 for H4 status and got to know it was approved by January itself. We request for H4 approval and letter by 03/27.

USCIS is not telling us the status of H1B as we are beneficiary only. My company immigration is telling they are working with USCIS team. I share the scanned copy of the H4 approval itself. My I94+240 days is expiring by 4/28.

Could you please help in this regard.
