H11B RFE to H4

Hello

My Visa Situation:
Valid H1B until 2020 with approved I-140

My Spouse Visa situation:
H1B extn is in process with RFE issued on 1/20/19. I-94 expired on Nov/10/18

Question:
1. If my spouse H1B gets rejected after RFE response, what option does she have to stay in US?
She is pregnant and prefer not to travel.
2. Can H1B to H4 COS possible with her expired I-94 through my company or through outside attorney?
3. Since I have approved I-140, is it possible to apply H4-EAD for my spouse.
4. Any other options?
