Extension of BLANKET L1A & L2( Spouse n kid)

hi ,

i have couple of query's to ask for extension of Blanket L1 A & L2 visa

1) Do i need to exit UNITED STATES for my extension of visa  or it can be done in United States . (presently i m in United States )

2) do i need to file DS-160 form again and consular appointment  , currently in United States

3) We dont have any international travel plans in 2018 & 2019

 

could you please update me for the same
